East Liberty Medical Clinic: Medical Physician Policies
- A valid health card is require for all OHIP visits. Without a valid OHIP card charges may apply.
- Patient’s are seen on a first come, first served basis for walk in services. We reserve the right to stop taking walk in patients once we have reached capacity for the day.
- No lab or test results will be given over the phone.
- You will not be contacted if lab results or tests are negative.
- Prescriptions are not filled or refilled over the phone. You are required to come in person to obtain any repeats for medications.
- No narcotics or controlled substances will be prescribed at this clinic.
- For emergency issues please go to your local ER department.
- The clinic hours are subject to change without notice.
- Not all services are covered. Please enquire within.
- Laboratory services may require a wait time. We will try our best to accommodate you the day of but this cannot be guaranteed. For fasting blood work please book an appointment with the receptionist once the doctor has given you a lab requisition form.
- We do not take transfer of complete charts from your previous physician(s). Certain documentation may be required for optimal health care, this is at the discretion of the physician.
- The physician will try to accommodate more than one issue per visit, but it is preferred to limit each visit to one main issue. This will help the overall flow and allow you to be seen in a timely manner.
- No medical advice will be given over the phone or by email.
- Booked appointments will be offered with certain physicians. Please inquire within. If there is a cancellation for booked appointments they need to be made at least 24 hours in advance, by phone or in person, with a live individual. Answering machine messages will not be accepted. Cancellations less than 24 hours may be subject to a fee.
- Referrals to specialists will be handled as promptly as possible and depend on the acuity of the situation. However, due to the specialist’s waiting list, there is no guarantees as to when you will be seen by them. If you wish for an earlier appointment with a specialist, it is best you call their office directly for openings or cancellations.
- If you have forms for the physician to fill out, filling them out are at the discretion of the physician. Charges may apply.
- Physicians can only write prescriptions for the patient they see. They cannot write ones for family members or friends without seeing them.
- All information between you and the physician is confidential. If you wish to release information to providers external to the clinic, please sign the consent forms to do so.
